NOTICE OF PRIVACY PRACTICES
THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Effective date: October 27, 2026
YouAgainHealthCo LLC (doing business as You Again Health Co.) provides telehealth services. We are required by the federal HIPAA Privacy Rule to protect the privacy of your protected health information (PHI), to give you this notice, and to follow its terms.
How we may use and disclose your information
- We share PHI with the medical director and others involved in your care, and with pharmacies and laboratories that fill your prescriptions and process your tests.
- We use PHI to bill and collect payment for your care, including through our payment processors.
- Health care operations. We use PHI to run our practice, such as quality review, training, record keeping, and compliance.
- Required or permitted by law. We may disclose PHI when required by law, for public health activities, to report abuse or neglect, for health oversight, in response to a court order or lawful legal process, to avert a serious threat to health or safety, and as otherwise permitted by HIPAA.
- Business associates. We use vendors who handle PHI for us, including our electronic health record and patient portal (Charm Health). They sign agreements requiring them to protect your information.
- Your authorization. We need your written authorization for uses and disclosures not described here, including most uses of psychotherapy notes, uses for marketing, and any sale of PHI. We do not sell your health information. You may revoke an authorization in writing at any time, except to the extent we have already acted on it.
Your rights
- You may inspect and get a copy of your medical and billing records, in paper or electronic form. We respond within 30 days and may charge a reasonable cost-based fee.
- You may ask us to correct information you believe is wrong or incomplete. We may deny the request in certain cases and will explain why in writing.
- Accounting of disclosures. You may ask for a list of certain disclosures we made of your PHI in the six years before your request.
- You may ask us to limit how we use or share your PHI. We are not required to agree, except that we must agree to a request not to disclose PHI to a health plan for a service you paid for in full out of pocket.
- Confidential communications. You may ask us to contact you in a particular way or at a particular place, and we will accommodate reasonable requests.
- Paper copy. You may ask for a paper copy of this notice at any time, even if you agreed to receive it electronically.
- Breach notification. We will notify you if there is a breach of your unsecured PHI, as the law requires.
To use any of these rights, contact us using the information below.
Our responsibilities
